BFR vs. KAATSU: A Side-by-Side Comparison
KAATSU and Blood Flow Restriction (BFR) training belong to the same broad category, but they are not identical methods. KAATSU pioneered the concept of modifying blood flow in the arms and legs to produce physiological changes, while many modern BFR systems use different cuffs, pressure protocols, exercise structures, and approaches to occlusion.
We often get asked whether KAATSU is BFR.
The honest answer is that KAATSU is the original method that helped give rise to the broader BFR market. The two share a category, but they differ in equipment, pressure protocols, research origins, historical safety records, contraindications, and how sessions are performed.
Those differences matter enough that we prefer to lay them out directly rather than leave customers to piece them together.

The terminology can create confusion because very different devices and protocols may all be described as “BFR.”
How Do BFR and KAATSU Work Differently?
Conventional BFR
BFR, as it is commonly taught and used in American physical therapy settings, often involves a wide, inflexible cuff inflated to a percentage of the user's measured limb occlusion pressure.
Pressure is then maintained during the prescribed training or rehabilitation bout. The goal is to impede arterial inflow to a prescribed degree while restricting venous return during the exercise.
KAATSU
KAATSU uses a narrow pneumatic band containing an oval-shaped internal air bladder.
Rather than maintaining one constant pressure throughout the session, KAATSU incrementally increases pressure across 30-second intervals and then deflates and reinflates rhythmically.
Under the KAATSU method, arterial flow is intended to continue throughout. Venous return is briefly slowed rather than continuously occluded.
This distinction is also reflected in the name. The Japanese term KAATSU translates approximately to “additional pressure” in English, rather than “blood flow restriction.”
How Do Their Pressure Protocols Differ?
Pressure is one of the clearest differences between conventional BFR and KAATSU.
Many BFR protocols set cuff pressure at approximately 40% to 80% of a user's measured limb occlusion pressure and maintain that pressure during exercise.
KAATSU does not begin by prescribing pressure as a percentage of limb occlusion pressure. A KAATSU session begins from the user's baseline condition and progresses incrementally using relatively low pneumatic pressure.
Within KAATSU protocols, continued arterial inflow is monitored through signs such as a detectable pulse, pink or rosy coloration in the limb, and rapid capillary refill.
The practical distinction is that conventional BFR commonly starts by determining how much pressure is needed to produce a specified degree of occlusion, while KAATSU is designed around incremental pressure without eliminating arterial inflow.
Do BFR and KAATSU Use the Same Repetition Protocol?
No.
The four-set 30/15/15/15
KAATSU protocols instead commonly use multiple sets in which repetitions decrease naturally from set to set as fatigue develops.
Depending on the exercise and protocol, KAATSU may use approximately three to seven sets rather than requiring the same predetermined repetition count for every subsequent set.
The distinction matters because a cuff or band alone does not define the training method. Pressure, timing, rest periods, repetitions, and exercise intensity are all parts of the protocol.
How Does the Research History Compare?
Both KAATSU and modern BFR draw on overlapping physiological research, but their research histories are not identical.
KAATSU's clinical history dates to the 1970s. Its documented research includes work associated with the University of Tokyo and more than 12,000 patients who reportedly underwent KAATSU as part of a decade-long research effort at the University of Tokyo Hospital between 2004 and 2014.
A foundational paper involving Yoshiaki Sato's method was published in the Journal of Applied Physiology in 2000. Later researchers in the broader BFR field frequently cited that early work.
The research literature specifically surrounding newer BFR protocols expanded substantially during the following years, including work in the United States on mechanisms, limb occlusion pressure, exercise prescription, and practical protocols.
For that reason, a study labeled “BFR” should not automatically be assumed to have tested the KAATSU method. The specific equipment and protocol used in the study matter.
How Does the Safety Approach Differ?
Both BFR and KAATSU have published safety records when appropriate protocols and patient screening are used.
KAATSU approaches pressure differently from sustained high-pressure occlusive systems. Its use of intermittent inflation, along with the protocol's requirement that arterial inflow remain present, is intended to reduce exposure to the risks associated with sustained high-pressure occlusion.
KAATSU protocols also use measures such as capillary refill and other observable signs to help confirm continued blood flow into the limb.
This does not mean that any blood-flow-modifying method is risk-free. Equipment, pressure, exercise intensity, health status, screening, and correct use all matter.
How Does the Equipment Differ?
BFR equipment varies considerably.
Some products are essentially manually tightened straps, cuffs, or tourniquets. Others are pneumatic systems that use wider cuffs and electronically controlled pressure.
KAATSU uses narrow elastic pneumatic bands connected to a controller. Its Cycle mode is designed to repeatedly and incrementally inflate and deflate those bands rather than hold them at one continuous pressure.
